In consideration of my child’s participation in the field trip to the James K. Polk Home & Museum on Tuesday, March 11th, 2025, I, the undersigned, hereby acknowledge and agree to the following:
- Assumption of Risk: I acknowledge that participation in the field trip involves inherent risks, including but not limited to injury, illness, or property damage. I voluntarily assume all risks associated with this activity, including those arising from travel, participation in guided tours, and other related activities.
- Release of Liability: I hereby release, discharge, and hold harmless Hidden Bee Learning Enrichment, its staff, volunteers, and any affiliated persons from any and all claims, demands, or causes of action arising out of or in connection with my child’s participation in this field trip, including any transportation to and from the event.
- Medical Emergency Authorization: In the event of an emergency, I authorize Hidden Bee Learning Enrichment staff or chaperones to seek medical treatment for my child as deemed necessary. I understand that in such cases, I will be notified immediately, and I accept responsibility for any medical expenses incurred.
- Photo & Media Release: I grant permission for my child’s image, voice, or likeness to be used in photographs, videos, or other media taken during the event for promotional purposes related to Hidden Bee Learning Enrichment, unless otherwise requested.
- Responsibility: I agree to ensure that my child follows all safety rules and guidelines established by the organizers, chaperones, and museum staff. I understand that failure to adhere to these rules may result in my child being asked to leave the event.
